So you should just ask for a large settlement. I don't know if they can possibly do that with it being in the future. Ask your lawyer what the law states about future compensation without a lump sum. If you get an award or settle a case based at least partly on this amount, you may have to pay back your carrier. Very clearly, you no longer are living a life free of pain, you probably can't do all the things you used to do before the accident, and you now have limitations. You probably will suffer loss of future earnings as well. I would suggest checking a jury verdict reporter for the jurisdiction you are in. See what juries have awarded to people who have suffered the same injuries as you, and that will provide a good benchmark from which you can formulate a base settlement demand before adding actual out of pocket costs and lost earnings.
