Premises Injury Question With Nearing 2 Year Deadline?

Nearly two years ago, I stepped in a hole on a Steak & Shake parking lot, injuring my hand. I immediately reported the incident, completing an accident form w/the mangager who provided a copy as well as a written statement that S & S assumed all liability for the accident & related injuries.
I was travelling at the time with an infant and my mother unable to walk alone. Otherwise, I would have immediately gone to the ER. I finished the 3 hour drive back to metro chicago and went to urgent aid the next day. No broken bones or sprain; simply bruising, pain & 2 missed days of work.
I obtained information from S & S regarding their insurance company & process to file a claim. Contacted the insurance company, Zurich, advising of my accident, injury & doctor visit. They assured me they would take care of the medical bills.
I recently received notice from a collection agency that Zurich did not pay & I am now responsible for that bill. What is my recourse just shy of 2 yrs?
