Are you a US Citizen? If so, you just need your passport, nothing else.

If you are from Europe, you could use your identification card that every european should carry. It acts as a passport in Europe.

What nationality are you? Please add this info to your question.
Proof of medical insurance is usually only required when you need to apply for a visa beforehand.
But regardless whether it is required or not, I highly recommend getting international health insurance. Anything else would be stupid. It is especially important that your health insurance covers emergency medical transport back to your home.
Other travel insurance is usually not necessary. Check the fine print of any insurance offered to you and then decide what you need. BTW, some credit cards already include basic travel insurance (but not international health insurance).
And if you are a EU citizen and covered by your national health care system you should be covered in the Czech republic too. I’m German and I have a little card saying “European health insurance card” in German. It has my German security insurance number, some other personal data and the name of my insurance society. Get one of those too if you can.
