Many medical procedures are much cheaper in places like India or Thailand. Why not have an option for people who are willing to travel overseas for medical treatment. If I can pay only 10% of what a procedure would cost in the United States that would save a lot of money. Insurance companies ought to be willing to insure me for a lot less money if I am willing to go to Thailand for an operation.

My company takes care of overseas dental treatment (medical tourism) and it is true that prices are a couple of times cheaper than in the UK, Ireland or in the US. Beside this, the quality of the procedures is on a very high level.
As it comes to refunds, for example in Ireland, the national health service refunds 20% of the treatment done abroad (20% tax relief). So why not to take a chance?

Well there are a few things wrong with this picture. First if the insurance company thinks you may have a condition that requires expensive medical procedures whether in the US or abroad they will charge you higher premiums or just not cover you at all. Many insurance companies would love it however if you paid the same premiums but then decided to go out of the country for your surgery. My company works with many insurers who will cover surgeries that are done in certain hospitals in mexico, costa rica, thailand, india, and a few other places.
Its a great thought but they will always continue to get as much out of you as they possibly can because they know you need them.
Medical tourism gained a footing in the US system a while back.
